Define

Problem statement. What is wrong with the testing process, where, since when, and how big is it? No causes or solutions yet.

Goal. The measure, its baseline, the target, and the date. For example: "Reduce the flaky test rate in the team's browser suite from 8% of runs to under 2% of runs by hour 305.5."

Customer and critical-to-quality requirement. Who suffers from the problem (developers, testers, clinicians, patients), and what they need.

Scope. In scope, and out of scope.

SIPOC.

Measure

Measure Operational definition Data source Baseline Target

Data rules: synthetic test data only; no real patient data; no personal data about colleagues beyond what the measure needs.

Analyse

Root causes, with the evidence for each (Pareto chart, fishbone diagram, five whys, or a hypothesis test):

Improve

The improvement, how it was piloted, and the result against the baseline:

Control

The control plan: what is measured from now on, by whom, how often, the control limits, and what happens when the process goes out of control. Who owns the process after the project.
